Ginkgo BilobaGinkgo biloba is one of the world's oldest living tree species, believed to have survived for 200 million years. Individual trees have lived 1000 years. Ginkgo trees are tall hardy trees, highly resistant to pollutants and pests with distinctive fan-shaped leaves. Ginkgo is considered a sacred tree by the Chinese and has been used in Oriental Medicine since ancient times for respiratory ailments and for brain function. Gingko has a long history of use in Chinese herbalism. Ginkgo seed is used in treatment for asthma, chronic bronchitis, spermatorrhea, leukorrhea, and nebulous urine. Ginkgo leaf is used in treatment for coronary heart disease and angina pectoris.The main active compounds in Ginkgo leaves are the flavo- glycosides: kaempferol, quercetin, isorhamnetin, and proantho-cyanidins. These compounds have anti-oxidant and free radical properties. Also important are the terpenes: ginkgolides and bilobalides. One way these agents decrease inflammation is by inhibiting Platelet Activating Factor (PAF) and reducing the stickiness of platelets which can result in decreased circulatory flow. PAF has been implicated in a wide variety of diseases including asthma, heart arrhythmias, myocardial infarction, and atherosclerosis. According to the American Botanical Council pamphlet on ginkgo, Ginkgo preparations currently available on the world market include dried leaf, tinctures, homeopathic preparations and various extracts. The leaves are a rich complex of chemical structures. Active constituents include bioflavonoids, such as the flavonoid glycosides kaempferol, quercetin and isorhamnetine; flavones; and organic acids. Since large amounts of gingko need to be used to get a pharmacologic effect, it is best used in extract form. For the elderly, Ginkgo may improve concentration and memory, absent-mindedness, headaches, and tinnitus (ringing in the ears). It may also aid leg circulation. A recent article published in the Journal of the American Medical Association confirms that Ginkgo Biloba Extract is an important new tool in treating Memory Loss and Dementia (Journal of the American Medical Association, October 22/29, 1997-Vol 278: 1327-1332.) An analysis of a 52-week placebo-controlled, double-blind, randomized trial of an Extract of Ginko Biloba for dementia by Pierre L. Le Bars, MD, PhD; Martin M. Katz, PhD; Nancy Berman, PhD; Turan M. Itil, MD; Alfred M. Freedman, MD; Alan F. Schatzberg; for the North American EGB Study Group concluded that EGb was safe and was capable of stabilizing and, in a substantial number of cases improving the cognitive performance and the social functioning of demented patients for 6 months to 1 year. Results from from 309 patients (mildly to severely demented outpatients with Alzheimer disease or multi-infarct dementia, without significant other medical conditions ) included in the intent-to-treat analysis: the EGb group had an ADAS-Cog (Alzheimer's Disease Assessment Scale-Cognitive subscale) score 1.4 points better than the placebo group (P=.04) and a GERRI (Geriatric Evaluation by Relative's Rating Instrument) score 0.14 points better than the placebo group (P=.004). The same patterns were observed with 202 provided evaluable data in which 27% of patients treated with EGb (120 mg/d) achieved at least a 4-point improvement on the ADAS-Cog, compared with 14% taking placebo (P.005); on the GERRI, 37% were considered improved with EGb, compared with 23% taking placebo (P=.003). For a herbal supplement, Ginkgo Biloba Extract (GBE) is pretty well studied. There are more than two dozen double blind, placebo controlled trials in humans and quite a few animal studies. There are no reports of adverse effects in the scientific literature; no reported toxicity; rare experiences of gastric upset or headaches. According to the Herb Research Foundation, the effective dose appears to be around 120 mg to 160 mg per day. This figure refers to the dosage of flavones and ginkgolides, that is, the main substances, not to raw herb or entire extract. It can take up to 6-8 weeks for cognitive effects to be noticed.
